In February 2023, we introduced an out of pocket fee will apply for some patients for routine consultations. In order to minimise the impact of this change, we will continue to bulk bill (i.e. not charge):

  • All children under 16 years of age as they are eligible for the bulk billing incentive*
  • All patients 70 years of age and over*
  • Patients eligible for the bulk billing incentive with a Centrelink Concession Card, Pensioner Concession Card or Commonwealth Seniors Health Care Card
  • Veterans with a Department of Veterans Affairs (DVA) white or gold card
  • Aboriginal and Torres Straight Islander patients*
  • Specific services such as chronic disease care plans and mental health care plans and health assessments
  • Any patient with a current Covid infection
  • Annual Heart Health Checks**

Vaccines available at Hills Family Practice

For children 5-11 years of age, please discuss with a doctor or nurse in order to obtain a suitable vaccination.
If you receive your vaccine elsewhere, please let us know when you next have a visit so that we can record this in your medical record at our practice.

What is a medical home?

There are many definitions of a medical home that incorporate general practice. We have visited and studied many systems around the world to learn about the best models of care, including Oregon, Cambridge Health Alliance in Boston, New Zealand, and the UK. However, we believe that a medical home is more than just usual general practice.

We define a medical home as a general practice which has embedded systems that enhance and deepen the relationship between the patient, GP, and practice team.

We are striving to become a comprehensive medical home by:
