Chronic Condition Management

Many people ask us about chronic condition management and “care plans”.

The Commonwealth Government has recently updated what were previously known as Care Plans. These changes came into effect 1 July 2025.

If you are living with a chronic condition, these changes are designed to support you in setting your own health and wellbeing goals. As your regular general practice, we will check with you every three months to review your plan and ensure your care stays on track. These review visits will continue to be bulk billed. A reminder that we encourage all of our patients to register with us in MyMedicare.

Flu Vaccines Now Available

Flu vaccinations for the 2025 winter season are now available. Government funded (free) vaccines are available for patients 6 months to 5 years of age, those 65 years and over, and patients with chronic health conditions as outlined on the NSW health website. All other patients will incur a private fee of $25 for their vaccines.
If you only require a flu (+/- covid booster) vaccination, we will have dedicated vaccine clinics available to be booked online or via our reception team. These clinics do not incur any out of pocket consultation cost for any patient. If you need to see your GP for other reasons in addition to vaccination, please book a standard or long appointment (routine fees may apply).
